The Extra-Mural Program (EMP) Congestive Heart Failure (CHF) and Chronic Obstructive Pulmonary Disease (COPD) Readmission Rates indicator is being measured to track the percentage of EMP patients who were discharged from hospital within the category "CMG 139 - COPD" or "CMG 196 – Heart Failure" without angiogram who are readmitted to hospital 30 days post discharge.
