About Continuing Care

The care/service received in the community to prevent, control, or relieve complications and/or side effects and to improve the citizens' comfort and quality of life.

This includes home care services (Extra-Mural Program and Home support services), long term care (nursing homes, special care homes and adult residential facilities) and palliative care services.
