The American Hospital Association's compendium of case studies details hospital-community partnerships that have addressed priority health needs in their communities. The hospitals and communities vary in location, service type, type of partners and degree of partnership. Insights gained from site visits to the communities helped with the creation of a companion guide, A Playbook for Fostering Hospital-Community Partnerships to Build a Culture of Health. Case studies addressed the following priority needs:
- Chronic disease management;
- Healthcare access;
- Mental and behavioral health;
- Obesity;
- Preventive health practices;
- Social determinants of health;
- Substance use disorders; and
- Violence.
