B.C. healthcare workers offer experiences from the field of team-based care

BC Team-Based Care is holding webinars with representatives of team-based care networks to share their work and experiences withing the Community Health Centre Evaluation in BC. The next webinar will cover:

1. How an evaluation tool was implemented and utilized in community health centres across B.C.;
2. The effectiveness of an evaluation tool by learning from its challenges and successes;
3. Relating their experience(s) with team-based care to others working in primary and community care teams; and
4. Identifying opportunities for strengthening team-based care at webinar participants’ local sites.
