The centres will aid hospital discharge across England to prepare services for winter. The centres host NHS, social care, housing and voluntary services staff in one place with the aim of locating and coordinating the best and quickest discharge options for patients, whether that’s at home, in social care or community care. NHS England plans to have the centres operating in every area by winter to reduce the number of patients who are medically fit to be discharged but who remain in hospital.
