Ontario hospitals adopt system to make patients' discharge process easier for them

The top two complaints reviewed Ontario’s Office of the Patient Ombudsman was from by patients with concerns over their discharge process. In response, 27 hospitals in Ontario are adopting a tool named the Patient Oriented Discharge Summary (PODS) to help patients better manage their post-hospital care and potentially alleviate stress for patients and their caregivers. PODS was co-developed by University Health Network’s OpenLab, patients and providers. It aims to provide patients with a set of clear, easy-to-understand instructions upon discharge to make the transition home easier and improve patient adherence to post-hospital care plans. It includes an individualized discharge process for each patient.
