Their Putting Patients at the Heart (PPATH) Program was awarded the Canadian College of Health Care Leaders, 3M Health Care Quality Team Award – Improvement Across a Health System. The award recognizes innovation, quality and teamwork for the purposes of fostering a better healthcare experience for patients. The PPATH program streamlines and simplifies access to follow-up care for cardiac patients to help them return home sooner and with better supports. The PPATH team engaged with cardiac patients, family caregivers, providers and physicians to redesign the healthcare journey (patients now report a 98% satisfaction with the program). Trillium Health Partners’ regional cardiac surgery centre has the second highest cardiac volumes in Ontario. It says PPATH saves the health system $950,000 annually and reduces post-surgery ED visits by 28%, and hospital re-admission rates by 38%.
