P.E.I. patient medical homes already full but showing promise

A dozen patient medical homes are up and running or in the developing stages on P.E.I., though Health P.E.I. says they’re not taking on new patients because they’re already full practices. The patient medical homes include at least one doctor and a team of healthcare workers, such as nurses, physiotherapists and dieticians. They were touted as a way to alleviate pressure on P.E.I.'s healthcare system. The province expects the collaborative approach to make it more attractive for doctors and healthcare workers to practice on P.E.I. A dietician says the model usually leads to faster referrals.
