A program aimed at helping vulnerable seniors and immigrants experiencing language and cultural barriers to recover after a stay at the hospital was launched in Brampton, Ont. The post-discharge services program uses visits and phone calls from staff to ensure low-income seniors are not isolated because of their inability to speak English. The staff will, among other things, help the clients connect to appropriate resources in their community. The initiative was a pilot project last year, it's being expanded as a result of three-years of funding of over $457,000 from an Ontario Trillium Foundation grant.
