The Health Quality Council (HQC) partnered with health systems and patient/family advisors to develop this survey to help better understand the patient/client experience in mental health or addiction services settings. The survey was designed to provide information for point of care managers and staff. The online survey includes a data analyzer tool and patient surveying toolkit.
