University of Illinois hospital integrates social determinants of health data into clinical workflows

As part of its mission to drive positive outcomes across the community, Illinois-based OSF HealthCare and nearby community-based organizations worked with two data software platforms. One, Pieces Decision Sciences, aims to improve outcomes within the hospital. The other, called Pieces Iris, delivers referrals to community-based organizations better able to provide the right level of care. The programs also aim to enable community-based organizations to collaborate. For example, a food pantry might use the system to refer individuals in need of employment to an employment assistance organization, while a social support organization might reach out to a nearby health system to evaluate medication adherence.
