The American Academy of Family Physicians’ screening toolkit was designed to help physicians recognize and respond to social factors that impact their patients' health. The toolkit includes a description of a team-based approach to screen for social determinants of health (SDOH), screening questions to define the patients' social needs (forms for physicians and for patients) and supporting resources to help physicians plan next steps to address any identifiable deficiencies. In 2018, the AAFP will expand on this first project with more resources by using community engagement and local advocacy initiatives. This community-level SDOH toolkit will include strategies for developing partnerships with local social and behavioral health resources to support a screening and referrals.
