Health systems and community stakeholders in some U.S. states are collaborating on initiatives to address social determinants of health. These health systems are finding that investing with community groups allows them to leverage those organizations' neighbourhood connections to increase community engagement in their hospital initiatives. With some health outcomes closely linked to income and other social factors, clinical community interventions have included health fairs and mobile mammogram and wellness screenings. Kaiser Permanente, for its part, says it's focusing its energy on connecting members to community-based providers that can address their social needs, but admitted that the social sector is under-resourced. It recently committed $200 million to start the Thriving Communities Fund to help reduce homelessness and housing instability.
