Chicago's Rush University implemented a social determinants of health (SDoH) screening tool that asks patients about housing, transportation, food security and utilities. Of an initial 50 patients screened, 33% indicated one or more needs, with food insecurity the most frequent (25%). Among patients with SDoH needs, 65% were on a hypertension registry and 30% were on the coronary artery disease registry. Rush built the SDoH screening form into its Epic EHR workflow to gain a better understanding of how the social issues connected to clinical measures and what they should be focusing on. Another step in the process involved working with a knowledge company to provide resources and services called a HealtheRx that matches the patient's needs and sends information to community-based providers and patients.
