This indicator is calculated by dividing the number of respondents who answered "always" by the number of respondents who answered "never, rarely, sometimes, usually or always" to the question: In the last 12 months, how often did the health professionals at the community health centre help you coordinate or connect the care from other healthcare providers and places when you needed it?
